Policies

Below you will find some of our office policies that are designed to make our patients feel at ease and provide you with the care that you deserve. If there is anything we can do to make your visit more pleasant, please notify us.

Office Hours

Monday - Friday 8:00am to 5:00pm
Evening and weekend call coverage

New Patients

New patients should arrive 30 minutes prior to their appointment time so that our staff may get your information registered within our systems. Please bring your downloaded patient forms with you to your appointment. We ask that you take the time to print and complete all of these forms and bring them with you to your appointment.

We also ask that you bring your current driver’s license and insurance card(s) with you; this will help ensure your insurance claim(s) are filed correctly. Please bring a list of the current medications you are taking as well as any previous diagnostic tests. If you require translation services for either limited English proficiency or if you are hearing impaired, please let our staff know so that we can make accommodations for you.

Appointment Times

We ask that you arrive for your appointments 15-30 minutes early for any paperwork that may need to be completed. Bring your patient information form with you so that we can streamline your experience. Furthermore, we suggest you schedule your follow-up appointment at the time of check out so you have a better selection of appointment times.

At the time of your appointment please bring:

  • Photo ID
  • Insurance card
  • Co-payments
  • List of current medications
  • Previous medical records (if you are a new patient)

Cancellations

We request a 24 hour advanced notice if you are unable to keep your appointment. If you are unable to give a 24 hour notice, it will count as a no show. By giving us ample notice, you are allowing us to fill the empty appointment time and prevent us from having to turn away someone who may need the appointment.

Payment for Services

Co-payments must be paid at the time services are rendered. If you do not have your co-pay with you, you might be asked to reschedule your appointment. It will be up to the doctor’s discretion.

Insurance

It is your responsibility to know what your insurance provides. Please read your policy and know its provisions as well as its exclusions. We will help you as much as possible with any questions you have regarding your insurance.

Before Seeing the Doctor

You will be seen by the nurse to answer a couple of questions to expedite your visit. This gives you the chance to get to know your nurse and ask any questions that you may have so that your doctor will be able to discuss them with you.

If You Require Surgery

We want to make you as comfortable as we can. You will have a pre-operative visit with your doctor so that you will understand the risks and benefits of surgery and have them answer any questions. At the same visit, you will then see our surgery coordinator, and she will go over your financial portion, consents, and will have blood work done if necessary.

Phone Calls

Due to the high volume of calls, phone calls are returned within 24 hours. If you’re having a problem, we will call you back within the same business day.

Privacy practices

Our practice is dedicated to maintaining the privacy of your health information. We are required by law to maintain the confidentiality of your health information. Please take time to read our privacy policy.
